Annual Grant Report Info
+ When should I submit my Organization's Annual Report?
Every organization, regardless of the type of grant, must submit an annual report to us within one year from the date of the award letter that accompanied the check. The report should be submitted by using the form below.
+ What if we have a multi-year award?
Annual Reports are still required regardless if the multi-year award has not yet been paid in full. A final report is due one year from the date that the final grant payment has been made and should include information about the impact of the entire multi-year award.
+ What if I haven't submitted a grant report but need additional funding?
It is our Foundation's strict policy to not review or award subsequent grant requests unless a grant report covering the prior award has been submitted.
+ What if my Organization requested an extension of our grant period?
If your organization submitted a request for an extension, and it was approved, than your extension is per that request and approval. Please include information regarding the extension in your report.
Grant Report Photos
Reporting Guidelines:
Grant reports are due annually regardless of the length of the grant period (i.e., multi-year).
Information should contain outcomes specific to the funds received in the reportable year.
If this is your final report after receiving a multi-year grant, please enter information specific to the entire grant amount.
All grant reports are due within one year from the most recent award letter and check date. You can save your progress on this form for up to 30 days by clicking the "Save" link at the bottom of the Form - if you allow 30 days to lapse, our staff will be unable to retrieve your information, and you will need to start over. You may also edit and print the form.
Upon submission, you will receive an e-mailed copy of your report. If you do not receive the form, please check your junk mail.

Reporting FAQ’S
We love to see the work that you are doing and the programs and projects that we have supported.
Please use the appropriate link below to submit any photos relevant to the grant funds you are reporting on.
Please ONLY submit photos relating to our grant - the photos should be of programs, projects, and/or visual reporting on what we funded.
Once you click on the link, you will see an "add photos" button at the top right of the screen.
Very Important: Please use the comment feature on each photo to tell us the name of the reporting organization.
If you do not, we will not know who submitted the photos.
Please comment on the photo using only the organization's name only.
If you received a grant in the past and want to show off those photos, please feel free to use the appropriate link to submit photos from past grants awarded by our Foundation.
We prefer organizations to upload their report photos to the Google site; if you choose not to upload photos to our Google Photos page but still want to show off your work, please e-mail at least three photos to photos@ctbfoundation.org after submitting your annual report.
Please do not e-mail photos if they have been successfully uploaded to Google, and please do not both upload them and e-mail them, one or the other. Since you can see that the photos were uploaded successfully, there is no need to confirm with us that you did. Thank you for your attention to this.
At the end of the grant reporting year, we create a photo book of all grantee photos for the Board and Staff to see. The book unveiling is our favorite time of the year. Thank you so much for sharing your project with us! Please use the appropriate link below:
